By using ClearIntake you agree to these terms. If you do not agree, do not use the service.
Not a medical device
ClearIntake is administrative software. It is not a medical device, it does not diagnose, treat, or prescribe, and its AI-generated summaries are clinician decision support only. A licensed clinician must independently review every patient before any care decision.
Patient use
You must provide accurate information at intake. Do not submit information about anyone other than yourself unless you are their legal guardian or authorized representative.
Clinician use
You may only access patient records for legitimate clinical purposes on behalf of your clinic. Sharing credentials, exporting records without authorization, or bypassing role and MFA controls is prohibited and may be reported to your employer and regulator.
Acceptable use
- No attempts to probe, scan, or attack the service.
- No automated scraping of patient or clinical data.
- No uploading of malware, illegal content, or content you have no right to share.
Availability and changes
The service is provided on an as-is basis. We may change, suspend, or discontinue features at any time, and we may update these terms; the “last updated” date will change when we do.
Liability
To the maximum extent allowed by law, ClearIntake is not liable for indirect or consequential damages arising from use of the service. Nothing here limits liability for fraud, willful misconduct, or anything that cannot be limited by law.
